Percutaneous Collagen Induction (PCI)
PCI or skin needling is a safe and minimally invasive alternative to more abrasive and invasive procedures. The simple technique involves a hand-held roller tool with rows of fine 1mm/1.5mm needles that create minute channels into the dermis, inducing a natural process of skin rejuvenation. Needling of the skin creates tiny areas of trauma that initiate a natural process of healing which releases numerous growth factors and also stimulates the production of collagen and elastin. As a result, the skin becomes smoother and tighter, without any damage to the overlying epidermis.
The epidermis is 0.2 mm thick and should at all times be kept intact to protect us from the environment and UV rays. Essentially one would aim for a thick, healthy outer layer of skin with collagen and elastin in the dermis and a good blood supply. The best way to achieve this is to feed the skin topically with vitamin A and antioxidant vitamin C, E and beta-carotene.
Topical vitamin A is well known for its skin normalizing effects, and, when it comes to wrinkles and sagging of the skin, the problems are in the dermis where collagen has been damaged and has become thinner. Percutaneous Collagen Induction is the ideal technique for improving the appearance wrinkles and shallow scars. The procedure is done with a topical anaesthetic.
How is PCI performed?

A topical anaesthetic is placed on the face and left to activate for 30 minutes. The face is then thoroughly washed with Sebuwash® to remove all the topical anesthetic. The skin is then rolled with the Medical Roll-CIT™ horizontally, vertically and diagonally. The skin is cleaned with clean swabs and EssentiA® is applied to the skin.
How long does PCI take?
It is advisable to set aside approximately 1hour for PCI; this includes preparation and treatment.
How does PCI work?

The revolutionary design of ENVIRON® medical needling devices allows for the efficient and safe creation of thousands of micro-wounds in the epidermis and dermis – a technique pioneered by ENVIRON® and known as Percutaneous Collagen Induction (PCI). Platelet release initiates a natural wound healing cascade that results in the synthesis of a confluent sheet of regenerative collagen and elastin as well as the breakdown and reorganization of scar tissue, which now heals under perfect conditions in contrast to those at the time of scarring.
Because high levels of vitamin A & C penetrate into the skin, more collagen is produced naturally (natural collagenesis). The high concentrations of vitamins A and C in ENVIRON® topical skincare products maximize collagen and elastin production and help to maintain the homeostasis between collagen formation following PCI and collagenolysis (collagen remodelling).
PCI utilises the natural wound healing pathway, even though the wound is miniscule. Each minute needle prick goes through the three classic phases of wound healing;
- inflammation
- proliferation (tissue formation)
- tissue remodeling
Each needle prick creates a micro-wound in the upper 0.5 mm of the dermis, causing some localized inflammation and bleeding. This in turn promotes the normal healing pathway of the body through the release of growth factors and infiltration of fibroblasts causing them to produce more collagen and elastin. Needling only penetrates through the epidermis and does not remove it, so the tiny punctures will rapidly heal. As a result of slight swelling, the holes close quickly. With thousands of minute punctures placed next to each other the effects virtually merge and a sheet of collagen will be laid down in the area just below the epidermis. Collagen slowly shortens after a few months and so the skin tightening is progressive.
How do I treat my skin after PCI?
You should take a tepid shower that evening and ensure that all the little bumps from the oozing are removed. Use AVST™ Vitamin A, C & E Body Oil or appropriate ENVIRON® recommended vitamin A skin care. Use a clean pillowcase, avoid direct sunlight and close contact with cats and dogs for a day or two.
How long will I take to recover?
Immediately after the treatment, the skin is slightly pink. Avoid using AHA’s for 1 day.You should be able to return to normal activities the next day. It is important to nourish your skin with the appropriate ENVIRON® recommended vitamin A products. Please discuss your specific requirements with your ENVIRON® Trained Professional.
How many treatments?
It is recommended to have a series of 6 treatments, one week apart depending on the condition of the skin. The series can be repeated within 3-6 months if desired.
What results could I expect from PCI?
PCI thickens and rejuvenates the skin; it can assist the appearance of uneven skin tones and diminishes the appearance of dilated blood vessels. The results of the treatment are ongoing and endure for many years, with the understanding that the long-term outcome is utterly dependant on and enhanced with the continued use of vitamin A, C, E and anti-oxidants to provide nourishment and protection to the skin. It is advisable to use a good sunscreen and sustain a health balanced lifestyle, to maintain great skin for a lifetime.
